Job Responsibilities:

As an RN in Home Health, you will be responsible for providing skilled nursing care and clinical support to patients in their homes. Your responsibilities will include:

1. Initial Assessments: Conducting comprehensive assessments of patients' health status, including physical assessments, medication reviews, and evaluation of home safety and support systems.

2. Care Planning: Collaborating with patients, families, and interdisciplinary healthcare teams to develop individualized care plans tailored to meet the unique needs and goals of each patient.

3. Wound Care: Performing wound assessments, dressing changes, and wound care management for patients with acute or chronic wounds, ensuring proper healing and infection prevention.

4. Medication Management: Administering medications, including injections, infusions, and oral medications, as prescribed by healthcare providers. Providing patient education on medication administration, side effects, and adherence.

5. Disease Management: Monitoring patients with chronic conditions such as diabetes, heart failure, COPD, and hypertension, and implementing disease management strategies to optimize health outcomes and prevent complications.

6. Patient Education: Providing comprehensive education to patients and their families on disease management, self-care techniques, lifestyle modifications, and symptom management strategies.

7. Pain Management: Assessing and managing pain in patients with acute or chronic pain conditions, including administration of pain medications, non-pharmacological interventions, and coordination of palliative care services as needed.

8. Documentation: Maintaining accurate and detailed documentation of all patient assessments, interventions, and outcomes in electronic health records (EHR) systems, ensuring compliance with regulatory standards and reimbursement requirements.

9. Coordination of Care: Coordinating with other members of the healthcare team, including physicians, therapists, social workers, and home health aides, to ensure seamless continuity of care and interdisciplinary collaboration.

10. Patient Advocacy: Advocating for patients' rights, preferences, and needs, and ensuring their voices are heard in the care planning process. Assisting patients and families in navigating the healthcare system and accessing community resources.

Requirements:

To qualify for this role, candidates must meet the following requirements:

1. Valid Registered Nurse (RN) license in the state of practice.
2. Bachelor of Science in Nursing (BSN) preferred.
3. Minimum of [X] years of clinical experience in nursing, with experience in home health, community health, or a related field preferred.
